This form is a Client Agreement between you, the client, and ShaynaMade©. By submitting this form, you are agreeing to everything mentioned within this form and our policies.

The goal of this form/agreement is to learn as much as we can about your design concept for this project. You, the client, are responsible for explaining the goals, content, and vision for your new logo design. The information that you provide to us within this form is what will be utilized to design your logo. Please take your time and fill out this form with as much detail as possible.

During the design process, our team will be bringing forth the design concept that you have explained in this design form. Once the design process begins, If you have a change of mind or want to completely change the original concept/idea, there will be an additional fee.
